The studio you gave your details to is responsible for them. ManagInk provides the software they use.
The health questionnaire you fill in before a tattoo: your name, date of birth, contact details, an identification document, and your answers about your health. Your signature and the date you consented are stored with it.
To carry out the tattoo safely and to keep the record a studio is expected to keep.
The number is recorded so the studio can confirm who you are and recognise you when you come back. It is hidden on screen and is not included in any list the studio exports. A member of staff has to ask to see it, and every time they do is recorded. It is stored scrambled, so it cannot be read out of the database itself.

The studio that tattooed you. It is not sold and it is not shared for advertising.
You can ask the studio for a copy of what it holds about you, ask for corrections, and ask to be erased. Erasing removes your name, contact details, identification document and health answers for good. The tattoos themselves stay in the studio accounts, with no name attached, because the studio has to keep its financial records. Every marketing email carries an unsubscribe link.
